Clergy Permission Cards
FROM THE DIOCESE OF LOS ANGELES: Sample clergy permission cards. Due to new HIPPA laws, clergy cannot be informed about a parishioner's helath issues in the event of an emergency without prior authorization from that parishioner or a responsible party. These are samples of cards that can be printed out on Avery 8871 labels and carried in a wallet or purse that grant permission for designated clergy to be informed in case of medical emergencies (1 page).
